The mission of the Chatham County Council on Aging is to promote and encourage independent living and physical and mental wellness through agency activities among the population over 60 years of age. Responsibilities of the agency include: (1) Providing home-based services, including in home aide, home delivered meals and minor home repair , that enable older adults to live independently in their own homes and avoid premature and costly institutionalization; (2) Providing health, wellness, nutrition, recreation and social programs and activities at both senior centers to support older adults living independently; (3) Offering quality information and referral programs to support older adults and caregivers in maintaining a high quality of life in the community; and (4) Collaborating with health and human services agencies, educational institutions,volunteers and professionals to extend health care access to underserved older adults throughout Chatham County.

This consumer-driven, coordinated system of information and services is offered by the Chatham-Orange Counties Community Resource Connections to minimize confusion, enhance individual choices, and strengthen individual decision-making pertaining to needs and supports.

Founded in 1949 in Durham as the Golden Age Society to provide neighborhood clubs for seniors, the agency received its first Older Americans Act grant in 1966, and officially changed its name in 1968 and remains an independent non-profit organization. With seven senior centers, including the downtown Durham Center for Senior Life, the Council offers a wide array of programs and services for older adults. Services include congregate meals, transportation, adult education, exercise classes, socialization, health promotion, caregiver support services, Senior Health Insurance Information Program (SHIIP), information referrals, and case assistance.

The mission of Family Support Network of North Carolina is to promote and provide support for families with children who have special needs. Family Support Network of North Carolina promotes and provides support for families with children who have special needs. Families are in a unique position to offer information and support to other families. An experienced family member can share the most practical advice and help a parent navigate the complex service system. Having support can make it easier for families to experience the joy and satisfaction that can come from parenting a child with special needs. This Parent-to-Parent support is available through local, affiliated Family Support Network programs across the state and through the Central Directory of Resources.

North Carolina Division of Social Services

North Carolina has a State Supervised, County Administered social services system. This means our State Government provides oversight and support, but it is the 100 local County Departments of Social Services that deliver the services and benefits. The main focus of the DSS is to protect and advocate for vulnerable populations—they have many programs and resources for both seniors and for parents of special needs children, including emergency assistance and food & nutrition assistance.

North Carolina Seniors' Health Insurance Information Program (SHIIP)

The SHIIP program is a free resource through the NC Department of Insurance to answer questions and counsel Medicare beneficiaries and caregivers about Medicare, Medicare supplements, Medicare Advantage, Medicare prescription drug plans, long-term care insurance and other health insurance concerns. The counselors offer free and unbiased counseling on Medicare health care products and provide assistance with Medicare/Medicaid billing errors.

Piedmont Health Services has been providing health care services to North Carolinians since 1970. They operate six community health centers and a SeniorCare center in Alamance, Orange, Chatham, and Caswell counties and have a service area of 14 North Carolina counties. Piedmont Health Services provides comprehensive health care, dental care, medical services, pharmaceutical, and nutrition services to all segments of the community, with special emphasis on those segments of the population receiving inadequate health care and whose access is restricted.

Resources for Seniors, Inc. (RFS) was founded in 1973 with a mission to provide home- and community-based services so that disabled and senior adults can maximize their independence for as long as possible while remaining in their homes. In 1993 the name of the agency was changed from Council on Aging of Wake County to better describe their guiding mission. Resources for Seniors operates programs and services for independent as well as dependent senior and disabled adults in Wake County.

UNC Home Health provides part-time, intermittent home health services to homebound patients. They provide intermittent skilled nursing care, home health aides that provide personal care for specific patient needs under the supervision of a registered nurse or therapist, and various forms of therapy in the home setting, including physical, occupational, and speech therapy. UNC Home Health primarily covers a service area of Orange, Durham, Alamance, and Chatham counties and is part of the renowned UNC Health Care System.

The ALS Association

ALS (Amyotrphic lateral sclerosis, often referred to as "Lou Gehrig's Disease) is a progressive neurodegenerative disease that affects an average of 15 new people each day...almost 6,000 people a year are newly diagnosed. The ALS association is the only nonprofit voluntary health agency dedicated solely to the urgent fight against ALS, The risk for ALS increases with age, so this affects both caregivers and seniors. The ALS Association relentlessly pursues its mission to help people living with ALS and to search for a cure to this degenerative disease.

American Diabetes Association

Autism Society of America
